• Required for participants in our Holiday Programs (does not include A+). One form per child is required.

A+ Afterschool Program Forms
Registration can be completed online or you can download and complete the A+ Printable Forms if you prefer. (All forms and payments must be completed prior to attending any program)
 

• A+ Printable Forms (includes the following forms needed to register for A+)
  • A+ Registration Form
  • Single Application for Child Care Assistance Overview
  • Application for Subsidized Monthly Fee (optional)
• A+ Authorization for Release of an Unaccompanied Child - Optional (one per child)
  • Authorizes your child to leave the program on his or her own.
• DREAM Co. Automatic Credit Card Billing Authorization Form for A+ - Optional (one per family)
  • If you would like to enjoy the convenience of automatic billing, simply complete this form. Upon approval, we will automatically bill your credit card on the first school day that an A+ payment is due (see forms for dates and more details).
